He is Suijakushin (a transformation of Yakushi Nyorai) and he was worshipped in Yasaka-shrine in Kyoto, This is why he is also called Gion Tenjin (Gion is the area that Yasaka-shrine is located in). He takes a variety of forms, however he commonly wears a cow head on his own 3-faced head, wears armor and has a wrathful appearance.
